Severe hand deformity in a patient suffering from rheumatoid arthritis.

Metacarpophalangeal luxation

Bones misalignment

Significant ulnar dislocation of all the metacarpophalangeal joints. This dislocation is also palmar clinically.

5th DIP luxation

Bones misalignment

Dislocated right 5th finger distal interphalangeal (DIP) joint.

1st IP luxation

Bilateral thumb interphalangeal joint dislocation.

Destruction

Rheumatoid arthritis

Round hypodensity

Carpal, metacarpal and phalangeal bone destruction and osteopenia tytpical of rheumatoid arthritis.

Pannus

Rheumatoid arthritis

Swollen wrist

Swelling of the periarticular soft tissue, particularly at the left radiocarpal joint, suggestive of rheumatoid pannus.

Carpal bone involvement

Important involvement of the carpal bones with bony destruction and collapse. On the left side, we can see a scapholunate dissociation with the scaphoid displaced on the radial side and the lunate displaced toward the ulnar side.

Sacpholunate diastasis

The significant scapholunate diastasis is measured at 8 mm (normally at 2 mm).

Capitatum migration

Proximal migration the capitate at the scapholunate diastasis.

Radiocarpal arthritis

Bone deformity

Degenerative changes of the distal radius, scaphoid and capitate. It is an advanced radiocarpal arthrosis.
