83-year-old female:
Hip pain.

Avascular necrosis

Irregular hypodensities

Cysts and sclerosis can be seen on the right femoral head. The relative absence of osteophytes despite the important sclerosis, however, is more typical of an avascular necrosis than of a severe coxarthrosis. The presence of cysts and sclerosis makes this a stage 2 femoral head necrosis according to Arlet and Ficat.

Crescent sign

A postero lateral crescent sign can be seen, that makes it an Arlet and Ficat stage 3 avascular necrosis.

Degenerative changes

Coxarthrosis

Joint space narrowing

Secondary coxarthrosis with severe joint space reduction
