Neer 1

Proximal humerus fracture

Linear hypodensity

Proximal humerus comminuted fracture. Transverse fracture line at the proximal humeral diaphysis (surgical neck).

Fracture of the greater tuberosity

Proximal humerus fracture

Bone deformity

fracture of the greater tuberositAvulsiony.

Displacement measurement

Greater tuberosity displacement is less than 1 cm and therefore not considered a "part" according to Neer's classification.

Angulation measurement

The angle of the humeral head is measured by drawing a line parallel to the humeral diaphysis, followed by a line parallel to the anatomic neck. The normal angle between both lines is 135°. Here, it is reduced to 112° (thus a 23° valgus deviation). Since the displacement is less than 45°, it is not considered a part according to Neer.

Proximal humerus fracture

Bone deformity

There is a nondisplaced lesser tuberosity fracture. As such, this is a Neer 1 type fracture despite the comminution.
