55-year-old male:
External rotation ankle trauma.

Maisonneuve fracture

Bone deformity

Right fibula undisplaced proximal shaft fracture. This is a Maisonneuve's fracture.

Syndesmosis

The force is transmitted upward through the syndesmosis starting at the ankle and toward the proximal fibula. As such, the syndesmosis is likely not preserved here.

Mortaise opening

Asymetry of the mortise (compare the space on both side of the talus) indicative of a syndesmotic injury.

Medial space measurement

Medial space to the talus is 6 mm (normal at 4-5 mm).

Tibiofibular overlap

The tibiofibular overlap is 8 mm (normal at 10 mm).
