Metacarpal fracture (4th)

Bone deformity

Diaphyseal oblique fracture of the 4th metacarpal bone with a small overlap and shortening of the 4th ray.

Angulation measurement

A slight 5° angulation can be measured on the lateral view. This angle may sometimes be difficult to measure due to the overlap with the other metacarpals.

Shortening

The 4th metacarpal bone is shortened. Normally, metacarpal length progressively increases from the 5th to the 3rd metacarpal (with the 5th being shorter than the 4th). Here, the 5th and the 4th metacarpal bones are of the same length, and the shortening is approximated to 2 mm.

Reverse Bennett fracture

The reverse Bennett fracture would be here.
