85-year-old female:
Fall from standing height on a hyperextended arm.

Proximal humerus fracture

Linear hypodensity

Surgical neck fracture with minor posterior displacement. According to Neer, this two-part fracture is a type 1.

Angulation measurement

There is a slight 5° posterior angulation.

Humeral head alignment

The humeral head seems to be adequately centered in the glenoid cavity, even though the transscapular view is imperfect. However, there is an upward migration of the humeral head, indicating a rotator cuff incompetence.

Acromioclavicular arthrosis

Reduced acromioclavicular space with significant osteophytic production.

Glenohumeral arthrosis

Cortical hyperdensity

Significant joint space narrowing with subchondral sclerosis of the humeral head.
