62-year-old female:
Shoulder pain following a fall from standing height on a hyperextended arm.

Proximal humerus fracture

Bone deformity

Proximal humerus comminuted fracture.

Normal angle

Varus/valgus angulation measurement

The 45° angulation of the humeral head is normal here.

Displacement measurement

Humeral head displacement greater than 1 cm categorizes this fracture as a Neer 2 type at the least.

Greater tuberosity fracture

Proximal humerus fracture

Linear hypodensity

A displaced fracture of the greater tuberosity can be suspected. The other shoulder views would be required to complete the evaluation.

Displacement measurement

The displacement of the greater tuberosity is greater than 1 cm, making it a part according to Neer. This fracture is therefore at least a Neer 3 type. Assessment of the lesser tuberosity on the other shoulder views is required to specify the Neer type of this fracture.
