85-year-old female:
Left thigh pain following a five stairs fall.

Femoral shaft fracture

Bone deformity

Distal femoral shaft spiral fracture shaft with a rotational deformity.

Posterior angulation

Anterior/posterior angulation measurement

20° posterior angulation.

Varus angulation

Varus/valgus angulation measurement

20° varus angulation.

Intra-articular involvement

The anteroposterior view doesn't include the knee joint, it is thus impossible to determine if there is an intra-articular extension of the fracture. On the lateral view, however, there appears to be an intra-articular extension at the posterior cortex level. This fracture line is readily seen on the postoperative radiographs, emphasizing the point that it is mandatory to take radiographs of the joints proximal and distal to any fracture.

Osteophytes

Round hyperdensity

Osteophytes on the back of the patella and at the femorotibial level.

Vascular calcifications

Round hyperdensity

Femoral and popliteal arteries calcification.
