54-year-old male:
High velocity car accident.

Pubic ramus fracture

Linear hypodensity

1st fracture line affecting the iliac crest.

Posterior column fracture

Acetabular fracture

Linear hypodensity

2nd fracture line affecting the posterior wall and column. This is therefore a fracture of the posterior wall and column.

Acetabular roof

The line of the acetabular roof is regular, indicating that the acetabulum is intact. This is assessed on the anteroposterior view.

Iliopectineal line

The iliopectineal line (evaluated on the anteroposterior view) is regular, indicating that the anterior column is intact. The iliac portion of the anterior column can also be seen on the iliac oblique view, whereas the inferior portion of the anterior column is assessed on the obturator oblique view.

Ilioischial line

The ilioischial line (evaluated on the anteroposterior and iliac oblique views) is interrupted, indicating that the posterior column is fractured.

Fractured acetabulum posterior lip

Acetabular fracture

Bony irregularities

Fractured acetabulum posterior lip (evaluated on the anteroposterior and obturator views).

Intact anterior lip

The acetabulum's anterior lip (evaluated on the anteroposterior and iliac oblique views) appears to be intact.

Acetabular teardrop

Fractured acetabular teardrop (evaluated on the anteroposterior view) representing the acetabular fossa.
